Procedures for United Tissue Network

 

  1. United Tissue Network will call us with a first call and we can also make an after-death referral to them. If they call us, they have already screened the body and accepted. If we are making a referral, we will need to screen the body and then make the referral to UTN. The family should not call! United Tissue Network will contact the family and conduct a medical history and social history interview with the Next of Kin and will make the decision for acceptance. United Tissue Network will call us with their decision for acceptance.  
  2. Once the first call is taken and entered in SRS, create a pick up notice and send over, along with the first call sheet to Dispatch at dispatch@carnesfuneralhome.com. If the driver arrives to where the decedent is located and finds that the decedent is decomposing and/or possibly unsuitable for donation for any reason, please call Dispatch and UTN ASAP!
  3. Keep the decedent in refrigeration until the scheduled transport. We DO NOT embalm!
  4. Obtain vitals from Next of Kin. Let them know that United Tissue Network will provide two (2) death certificates to the family. If they should need more, they will have to purchase them at $25 for the first copy and $15 for each additional certified copy. Enter vitals in TER and print a Facts of Death Verification Form by clicking on the Informant Verification icon at the top of the TER webpage. Email a copy to the Next of Kin to proof along with the death certificate request form.
  5. Obtain the Burial Transit Permit from the appropriate county clerk in which the death occurred by faxing over a Fax Cover Sheet requesting a Burial Transit Permit for Removal from State and a Facts of Death Verification Form. If the county is Harris County or City of Houston, you must send a driver in person to obtain the permit. They do not accept faxed requests. The appropriate registrar needs to be designated in order for them to generate it and the type of disposition must be checked as “Removal from State”.  Place of Disposition for this transit permit is United Tissue Network in Norman, OK or Phoenix, AZ. UTN will notify you which office the decedent will be transported to.
  6. Prior to transport, United Tissue Network needs to be contacted to verify if we are driving the decedent to Dallas or to the Oklahoma office. They will give you instructions for delivery.
  7. United Tissue Network does not accept bodies over the weekend. The transport has to be done during the week.
  8. Designate the doctor to sign in TER. Call the doctor to inform them they have been designated to sign.
  9. Once signed, order the number of death certificates the family wishes including 1 certified copy for United Tissue Network. Once they come in, email or fax a copy of the DC to UTN and the rest to the Next of Kin.
  10. Invoice United Tissue Network by emailing the invoice.

 

To Invoice:

The referral fee for United Tissue Network is $550. This includes the removal, holding of the decedent until transport arrangements have been made, transportation to the organization, permits and filing the death certificate, and 2 certified copies of the DC for the family. If we are transporting to UTN in Oklahoma, we charge $700, $200 extra for a 2nd body. If we are transporting decedent to Dallas, we charge $400, $100 extra for 2nd body.
